We work closely with hospitals, managed care plans, and community partners to assist members with housing applications, document readiness, transportation services, benefit coordination, and access to medical and social support systems. Our team helps individuals navigate barriers that may prevent them from obtaining stable housing by providing compassionate, person-centered support every step of the way. We meet patients where they are and care for the whole person — helping them transition from crisis toward stability, healing, and long-term independence.

Tenancy Supportive Services

Our Tenancy Supportive Services program is designed to help individuals successfully maintain stable housing and build long-term independence after placement. At , we provide ongoing support to help members adjust to independent living, navigate challenges, and remain connected to essential community resources. Services may include assistance with understanding lease responsibilities, communication with property managers, benefit coordination, transportation support, connection to healthcare and behavioral health services, life skills support, and ongoing case management.
